docs(plan): add nvim modernization implementation plan

Three-phase, single-branch plan derived from the modernization
spec. Each phase is one commit with restart-and-validate gates.
Phase 1 fixes critical bugs and removes abandoned plugins;
phase 2 swaps notify/dressing for snacks UI; phase 3 swaps
nvim-cmp for blink.cmp and telescope for snacks.picker.
